This form is used to provide affilliation verification for personal purchases of most restricted items. In lieu of this form, the purchaser may have their order shipped directly to their law enforcement agency or military installation. To proceed, fill out the information and include images of employment ID. These can be smartphone photos. Once received, this information is subject to verification. It is then securely filed for future purchases of restricted items. All information conveyed here is considered confidential.
